The 2004 mint set included only one insert about the uncirculated coins and did not include a re-order
card as seen in earlier mint sets. The US Mint placed the one insert in the Philadelphia envelope.
The front of the insert in the 2004 mint set included a simple design of charcoal gray
printed on a white background.
Though too pale to be easily seen in the following picture, an image of the heraldic eagle forms a background
for the title on the front of the Certificate of Authenticity.
Inside the 2004 insert, the US Mint discussed how they struck
the uncirculated coins, their mint marks and the introduction of the first two Westward Journey
nickels. In addition, they highlighted the continuing 50 State Quarters Program.
The bottom section inside the Certificate of Authenticity identifies the specifications for the five
new state quarter uncirculated coins including their designs, artists, metal composition, weight and size. The
quarters in the 2004 mint set included Michigan, Florida, Texas, Iowa and Wisconsin.
The back of the folded insert shows the coin specifications for the cent, "Peace Medal" nickel, "Keelboat"
nickel, dime, half dollar and dollar uncirculated coins.
For the 2004 mint set, the US Mint stopped including the re-order cards — perhaps due to their
improved web site and ordering procedures.
